Meritus Health staff members were recently recognized in three separate areas.
The outpatient care management team, alongside supportive departments and community partners, recently received a Circle of Honor 2021 Minogue Award for patient safety innovation from the Maryland Patient Safety Center, according to a Meritus Health news release.
The recognition was for reduction of patients readmitted from skilled nursing homes.
Improved communication, medication reconciliation, use of referrals and standardized workflows led to a reduction of readmissions from 50 in the first six months of 2018 to 30 in the first six months of 2019, according to the release. The patients were from eight local skilled nursing facilities
Meritus Health was one of eight hospital/health systems in the state to receive the recognition.
Meritus also honored its first two DAISY nurses of 2021, Anna Camp in critical care services and Kelly Robinson in intermediate and medical/surgical care.
Camp was nominated by relatives of two patients, while Robinson was nominated by her clinical manager.
The DAISY Award is an international program that rewards and celebrates nurses for their clinical skill and compassionate care.
In addition, two certified nursing assistants, Alexis Cook and Michelle Sonntag, were the most recently selected Champion Nursing Assistant award winners.
Cook was nominated by a patient in the women’s and children’s services unit. Sonntag, who works in critical care services, was nominated by a co-worker.
